All it takes to begin using our senior care services is to meet with us and verify your eligibility with a few necessary documents — and we take care of the rest.

Enrollment, Eligibility, and the Nemours SeniorCare Application Process
Eligibility Requirements
To participate in our senior care services, you'll need to meet three requirements:
- Age: 65 or older
- Residence: Delaware resident
- Annual income ceiling:
- Single = $26,355
- Married = $40,880 (regardless of spousal eligibility)
Application Process
To apply for Nemours SeniorCare services, you’ll need to follow the three-step process below. If you’re not eligible for our low- or no-cost Nemours SeniorCare services, there are services available for vision, hearing, and dental from other senior care facilities located in the Delaware area. Visit our additional resources section for a list of recommended providers.
1. Schedule Your Enrollment Appointment

2. Verify Your Eligibility
You will need to bring documents to your appointment that verify your age, residence, and income eligibility. See accepted documents in the previous section that covers “Eligibility Requirements.” One of our staff will interview you and go over your documents to see if you're eligible for our senior care services.
Other items to bring:
- Social Security card
- All medical and hospitalization cards (i.e., Medicare, Medicaid, prescription card, or other insurance cards)
3. Receive full membership benefits.
Once approved, you are now eligible for any and all of our senior care services and products. You'll receive a schedule of applicable co-pay charges.
- Your membership is valid for one year
- Bring photo ID with you to all of your appointments and for any services provided

- Social Security Form #2458

- Insurance policy
- Marriage record
- Bible or family record
- Confirmation certificate
One of the following:
- Current driver’s license
- State ID card
OR two of the following:
- Public utility record and receipts
- Business bills
- Post Office records
- Social agency records
- Rent receipts
- Voting record
- Social Security check copies
Single: $26,355
Married: $40,880 (regardless of spousal eligibility)
All types of income must be included and documented, such as:
- Social Security Income
- Interest income
- Pensions and IRAs
- Work earnings
- Dividends
- Annuities
- Rental income
Bank statements for all existing bank accounts are required at every membership appointment. Gross earnings from all pensions, IRAs, and work salaries are used to calculate total income.
